Here is how I fixed my MP4 made in China ? Purchased from Ebay, shipped out from Hong Kong.

In MP4 device:
go to SETUP -> ONLINE MODE -> select ?NORMAL NODE ONLY?, then back to main menu.

Connect MP4 to your PC.

Right click on ?Start? -> ?Explore? ->Removable Disk (Esmiley - (F: or G: whatever your MP4 driver), you should only see one driver, not two.

Right click on ?Removable Disk? ->format -> File system. Select FAT16 if your MP4 is 2GB or smaller, if big than 2GB then select FAT32. Also check the box for Quick format.

Hit ?Start?. After reformat the device, it should back to normal.
